CWP - Community Mental Health Team

The community mental health team will have a base where staff can review clients and conduct out patient clinics. They may also offer a range of other services such as psychological therapies, specialist investigations for monitoring of medication, social care assessments, assessments for respite and day care, etc. A regular (usually weekly) multidisciplinary meeting is held where staff discuss how best to help their patients. The staff members work closely together to ensure that every patient has an appropriate careplan to meet their needs.
