Advanced Nursing Service/Patient wrap around service

We are a group of twelve GP Medical Practices, located in the Durham Dales area of County Durham, who work together to provide high quality primary healthcare services to support the care delivery for frail, elderly people. The team is available to visit you, if you have been identified as being at risk of hospital admission, in order to provide a health assessment, medication review and to develop a care plan which includes your choice of preferred place of care. We have a multi-skilled nursing team to meet your needs and provide a seven day service.
