Hartlepool Community Mental Health Team

The Hartlepool community mental health team provides mental health assessments for people living in the Hartlepool locality who have memory problems or who have other mental health problems (eg anxiety, depression etc). The service has a memory clinic that assesses and diagnoses people with dementia. They provide ongoing support to the patient and their carerÍs, prescribe medication if appropriate and monitor their mental health. The team accepts people who are predominantly aged 65 and over. However if the patient is younger but it is felt that the service is best placed to meet their needs, the team will assess them.
