Alcohol Related Long Term Conditions Team

We are an alcohol related long term conditions service. We work with patients with a history of alcohol dependence, high risk of readmission to hospital and an ongoing alcohol related long term condition such as alcohol related liver cirrhosis, end stage liver failure, peripheral neuropathy and alcohol related brain injury. We coordinate care with the aim of reducing unplanned care.
